Is concentration of capital at the top income levels the cause of capitalist societies with no middle class?

Actually, if you look at the income tables over the last 20 years, much of the middle class has become the upper middle class (aka the "rich" to leftists)...I call that upward mobility and just how the free enterprise system should work.
